January 14, 3:30 PM. Matapos ang anim na session ng nakakatuwang root canal treatment nung nakaraang taon, kinakailangan nang tanggalin ang higit na nakakatuwang impacted na wisdom tooth. As what my attending clinician, Monica, would comment, "ang kulit ng ngipin". Hindi ko pa makuha sa simula kung bakit "makulit" ang ngipin pero sa tulong ng dental xray at matiyagang pag-explain ni Monica, na-realize ko na weird nga ang ngipin. Tilted, almost 90 degrees to the front.

Teeth #47 ito according sa code ng mga dentists.

January 25, 8:30 AM. Oral Surgery Section. Dumating ang araw ng paghuhukom. As warned by Monica the day before, odontectomy will often result in swelling of the cheek (parang mumps), instances of post-surgery pain and some bleeding. For the first time, parang nalagay ako sa mindset ng taong ooperahan kahit "minor" surgery pa ito. Added to the complexity of things is the fact that one of the inner nerves can be permanently numb if hit inadvertently. Sabi ko ok lang dahil "malayo sa bituka", pero kabado na ako.

Pag-upo sa dental chair, masaya akong binati ni Ma'am Jan, ang PGH dentist na mag-assist kay Mon. Everything seems to be in place, ayos! Pre-ops protocols, some briefing and the carnage starts. To simplify the procedure:

1. anesthesize
2. na-drill na upper portion para magka-access
3. parang ibinaba ang portion ng gilagid. may konting bone din yatang tinanggal
4. nag-anesthesize ulit sa inner pulp
5. na-partition ang ngipin to 4 parts



6. nag-puncture ng hole at ikrinak ang ngipin (hehe, parang nakikita ko)
7. hinatak ang unang partition, return to step 6, hinatak ang 2nd partition
8. madugo na at this point, in addition sa pagiging masakit. pero wala pa sa climax.
9. ini-extract ang base ng ngipin. Challenging dahil "twisted" ang pinaka-base.
10. hintak ang last partition. Pinakamadugo sa lahat dahil sumasabit sa laman.

Natapos ang hilahan ng wisdom tooth sa loob ng limang oras. A list of post-op guidelines where provided to anticipate the potential complications. Disoriented paglabas ng UPCD building. Nakakatuwa din dahil palagay ko, may mga "milestones":

1. probably, the most watched odontectomy in UPCD
2. na-introduce sa mga clinicians ang device na laxator
3. mabilis na healing. the day after, back to normal diet. at walang lump sa pisngi
4. joint dentist and clinician horizontal matress suture
5. IMHO, the most challenging yet fulfilling dental surgery for Monica.

Moral story: Ang buhay parang ngipin lang. Minsan diretso, minsan impacted. Ang mga solusyon ay maaring kasing simple ng paglilinis ng mess (i.e. scaling),pagtutuwid ng landas (i.e. orthodontics) o kasing hirap ng pagtatanggal ng threats (i.e odontec).
